Form & Function Fyzio, PLLC is an out-of-network provider with most insurers but is an in-network Medicare provider. Depending on the insurance plan, many providers cover out-of-network outpatient physical therapy services. In the state of Florida, Medicare with a supplemental or secondary typically covers services 100% (80% by Medicare Part B with the remaining 20% billed to the supplemental or secondary as per the Coordination of Benefits on file).
Many insurance companies reimburse for out-of-network physical therapy. Form & Function Fyzio, PLLC will gladly verify benefits and send a report of coverages and limits at no charge to the potential patient. “Superbills” for insurance providers can also be provided upon request.
Coverage, co-pay, and out-of-pocket deductible gimmickry is ever-changing, so it is difficult to provide concise and concrete guidance on what is best for any particular individual. The best bet is always to contact the insurer directly. HMOs are typically the worst at covering out-of-network services. If the plan is an HMO, be diligent in checking with your insurer with regard to the extent of coverages, but the simple question for the insurer is: “What coverage do you provide for out-of-network outpatient physical therapy services?”
For cash pay services, payment is due at the conclusion of each session. Not involving insurance companies allows full autonomy with regard to your healthcare decisions: without the restrictions imposed by “authorizations” or visit limits. The patient receives exactly what they deem necessary.
